Must have high school diploma or equivalent, valid driver's license and satisfactory driving record and good interpersonal skills. The Eligibility Specialist will communicate with consumers to ensure all financial forms are completed. They will also complete request for services form and all economic assessment forms for consumers at first contact. The Eligibility Specialist will determine copays and/or payments for outstanding balances and complete data entry from the service activity logs for all programs. Other related duties as assigned by supervisor.
